Monthly Nutritional Supplement

What it is

The MNS is for people on disability assistance with the PWD designation who have a severe medical condition causing chronic, progressive deterioration of health with wasting-type symptoms, and who need additional nutritional items or vitamins and minerals to prevent imminent danger to life. Current maximums (Health Supplements & Programs rate table, effective August 1, 2023) are $180/month for dietary items and $45/month for vitamins or minerals.

Who it is for

A practitioner must confirm at least two symptoms such as malnutrition, underweight status, significant weight loss, significant muscle-mass loss, significant neurological degeneration, deterioration of a vital organ, or moderate-to-severe immune suppression.

How to apply

1. Get form HR2847 (Application for Monthly Nutritional Supplement) from your ministry office, My Self Serve or the link above
2. Complete Part B yourself
3. Have a medical practitioner, nurse practitioner or registered dietitian complete Part C describing your condition, symptoms and the items needed
4. Submit the form; the ministry's Health Assistance staff review and decide

Practical tips

• A registered dietitian can complete Part C — often faster than waiting for a specialist appointment
• Be specific about each symptom: two or more must be documented for approval
• If denied, request reconsideration — MNS denials are frequently overturned with better medical detail
